Cohler Care

New patients

Welcome to Cohler Care. Here's what to expect, so your first visit is simple.

How it works

Whether you're in for one visit or starting a program.

Path A

One-time visits

Same-day sick visits, DOT physicals, sports and other physicals.

  1. Call to schedule (or walk in for sick visits): Call (928) 440-6880 to schedule. Same-day sick visits and non-DOT physicals (sports, school, camp, general): walk-ins also welcome during clinic hours. DOT physicals are by appointment only.
  2. Come in: Bring your photo ID and anything your medical provider requires, if it applies, such as clearance or records for diabetes, high blood pressure, or sleep apnea. For DOT physicals, also bring your CDL.
  3. See your provider: Robert handles the visit. Pay with a debit, credit, or HSA card. Starting at $49 sick visits / $75 physicals / $25 sports.

Path B

Ongoing care

Testosterone therapy, hormone pellets, women's hormone replacement, and medical weight loss.

  1. Call to schedule: Call (928) 440-6880 to schedule a $50 consult with Robert. For TRT and women's hormone replacement (non-pellet), the $50 is applied to your first billing if you sign up.
  2. Consult: Meet with Robert and fill out a questionnaire at the visit.
  3. Labs: Robert orders the required labs for treatment. Labs are your cost: bill them to your insurance, or self-pay through Access Lab or Fullscript.
  4. Review results: Once labs are in, Robert calls you back in, goes over your results, and talks through whether the program is right for you.
  5. Start & follow up: If it's a fit, you start (first dose in office / learn to self-administer where it applies), then ongoing follow-ups: for weight loss, a monthly visit to pick up your 4-week supply (medications are prescribed one month at a time and adjusted to your unique needs); for TRT, connect with Robert monthly as needed, plus labs and an in-person visit every 6 months (no visit fee).

Yup. It's really that simple.

Lab work

For hormone therapy and medical weight loss, Robert orders the required labs for treatment after your consult. You'll come back for a results visit before starting a program. Labs are your cost: bill them to your insurance, or self-pay labs through Access Lab or Fullscript. For TRT, the lab order is included in your monthly price. Self-pay labs through Access Lab or Fullscript typically run about $100–$300, depending on the panel; the price varies with the tests ordered.

Consult fee

Hormone therapy and medical weight loss start with a $50 consult. If you sign up for TRT or women's hormone replacement (non-pellet), the $50 is applied to your first billing. (It isn't applied to pellets.)

Questionnaire

You'll fill out a questionnaire at your consult with Robert. We don't email PDF forms. Please don't send health information by email. Call (928) 440-6880 to schedule.

New patient FAQ

Does Cohler Care take insurance?

No. Cohler Care is a self-pay clinic, so we don't bill insurance. We accept debit cards, credit cards, and HSA cards. Prices are listed as “starting at” amounts on our pricing page, and your provider reviews your exact cost with you before treatment.

Can I email you about my symptoms or other medical questions?

Please don't. Our clinic email is a regular Gmail account and is not a secure channel for health information. To discuss anything medical by message or chat, use the OnPatient app (from DrChrono), or call (928) 440-6880. You can't sign up for OnPatient yourself: we send patients an OnPatient invite once they're in our system. New patients, or anyone without an invite, please call (928) 440-6880. Appointments are booked by phone.

How do I book an appointment at Cohler Care?

Call (928) 440-6880 to make an appointment. Same-day sick visits also accept walk-ins during clinic hours. DOT physicals are by scheduled appointment only. New to the clinic? Read what to expect as a new patient.

When I call, why does it say Northern Arizona Health Club?

That's correct, you've reached the right place! Cohler Care is located inside Northern Arizona Health Club, so calls to (928) 440-6880 start with the health club's greeting. Choose the Kingman office to talk with someone from the Cohler Care team.

Do I need lab work before starting hormone therapy?

Yes. After your consult, Robert orders the required labs for treatment. Labs are your cost. Labs: bill them to your insurance, or self-pay labs through Access Lab (home-collected labs) or Fullscript. For TRT, the lab order itself is included in your monthly price. Once results are in, Robert calls you back in to review them before you start a program. TRT patients also have labs and an in-person visit every 6 months, for monitoring and prescribing regulations; labs are the only extra cost at that visit. Self-pay labs through Access Lab or Fullscript typically run about $100–$300, depending on the panel; the price varies with the tests ordered.

Do I need to fill out forms before my first visit?

You'll fill out a questionnaire at your consult with Robert — not beforehand through a portal. We don't email PDF forms, and please don't send health information by email. Call (928) 440-6880 to schedule.

How often are follow-up visits?

Medical weight loss patients come in monthly to pick up a 4-week supply of medication. Medications are prescribed one month at a time, and adjustments are made based on your unique needs. Talking with Robert about anything else costs nothing more. TRT patients can connect with Robert once a month as needed, and have labs and an in-person visit every 6 months for monitoring and prescribing regulations (no visit fee; labs are the only extra cost). Women's hormone replacement (non-pellet) patients follow up every 3 months, or more often as needed while fine-tuning medication and dose. Pellet patients have pellets placed every 3–4 months, and calls or visits with Robert in between are at no charge.

Is there a no-show fee?

Yes. There is a $25 fee for a no-call, no-show, billed to your card on file. If you can't make your appointment, please call (928) 440-6880.
